Diabetes Program - Clinician's

 
The Diabetes Program provides self-management classes to adults diagnosed with type 2 diabetes through our American Diabetes Association (ADA) recognized program.

Program Goals:
  • Collaborate with health care providers to improve client outcomes
  • Provide diabetes education to clients to improve their ability to self-manage
The Diabetes Program provides a comprehensive diabetes education program. The program is designed to enhance each participant’s level of diabetes self-care. The program offers a 7-week education class series in various languages (English, Cantonese, Farsi, Hindi, Punjabi, and Spanish) with emphasis on the following topics:

  • ABC’s of Diabetes

  • Meal Planning and Carbohydrates Counting

  • Portion Control and Label Reading

  • Medications and Blood Glucose Monitoring

  • Physical Activity

  • Hyper/Hypoglycemia and Stress Reduction

  • Sick Day, Complications, Graduation

Keeping the adult learner in mind, the sessions focus on applications of proper diabetes self-management. High-level interaction with the participants using their everyday examples provides a very dynamic group experience. The Diabetes Program believes in getting to the heart of what the community needs and teaching the community how to deal with diabetes in the real world.

Monthly support groups in Dublin, Fremont, Oakland, Livermore, Pleasanton, and San Leandro are provided to clients who participate in the program. Clients also receive a monthly newsletter.

Q. When and where are the diabetes classes offered?

A. Classes are offered throughout the county in various places and times so it is best to call to register for a class.

Q. How do I sign-up for diabetes classes?

Q. Can I attend a few of the diabetes classes?
A. It is recommended that individuals attend ALL the classes in order to completely understand and control your diabetes.

Q. Are diabetes classes offered in languages other than English?
A. Classes are offered in Cantonese, Farsi, Hindi, Punjabi, and Spanish.

Q. If I attend the diabetes classes, do I also have to attend the monthly support groups?
A. The monthly support groups are not mandatory, but they are a good resource for diabetes-related issues and/or concerns.

Q. How do you handle requests for services not covered by your program?
A. The program would refer them to an appropriate resource.

Treatment Protocols:


The Diabetes Program uses the Basic Guidelines for Diabetes Care developed by the California Department of Public Health, Diabetes Prevention and Control Program of California, and the Diabetes Coalition of California to determine treatment recommendations during the case management process.
