The Diabetes Program provides self-management classes to adults diagnosed with Type 2 diabetes. The diabetes team provides education classes, support groups, and develops a treatment plan to assist you in controlling your diabetes.
What is Diabetes?
Type 2 diabetes results from your body not producing enough insulin or your body not using the insulin properly. Type 2 diabetes develops in adults and more recently has been found in children. You may have diabetes and not know it, as often there are no symptoms.
You may be at risk for diabetes if you:- had diabetes during a pregnancy or your baby weighed more then 9 pounds
- your brother, sister, mother or father have diabetes
- are overweight
- have high blood pressure
- have high cholesterol
- get little to no physical activity
- over the age of 65
- are African American, Latino, Native American, Asian American or Pacific Islander
Type 2 diabetes can be controlled so you can live a long and productive life.
The Program’s Goals are to:- prevent medical problems related to diabetes.
- improve your understanding of type 2 diabetes.
- improve your ability to control your diabetes.

| Program Eligibility: |
Patients referred to the Diabetes Program must meet the following requirements to participate in group education classes and/or the case management program.
- Live in Alameda County
- Must be 18 years or age or older. (We will consider younger clients on a case-by-case basis.)
- Must be diagnosed with pre-diabetes or type 2 diabetes. (Individuals with type 1 diabetes will be admitted to class on a case-by-case basis.)
- Individuals providing care for those clients requiring assistance with activities of daily living are welcome to attend.

Diabetes Education The Diabetes Program believes in getting to the heart of what the community needs and teaching the community how to deal with diabetes in the real world. Our 7-week education session is offered in English, Cantonese, Farsi, Hindi, Punjabi, and Spanish. Classes cover these topics:
- ABC’s of Diabetes
- Meal Planning and Carbohydrates Counting
- Portion Control and Label Reading
- Medications and Blood Glucose Monitoring
- Physical Activity
- Hyper/Hypoglycemia and Stress Reduction
- Sick Day, Complications, Graduation
Frequently Asked Questions: Q. When and where are the diabetes classes offered? A. Classes are offered throughout the county in various places and times so it is best to call (510.383.5185) to register for a class.
Q. How do I sign-up for diabetes classes? A. Call us.
Q. Can I attend a few of the diabetes classes? A. We recommended that individuals attend ALL the classes in order to completely understand and control your diabetes.
Q. Are diabetes classes offered in languages other than English? A. Classes are offered in Cantonese, Farsi, Hindi, Punjabi, and Spanish.
Q. How many times will the diabetes case management staff visit my home? A. The number of visits depends on your needs and will be determined by the case manager.
Q. How do you handle requests for services not covered by your program? A. The program would refer them to an appropriate resource.
