Community Health Teams

 


Community Health Teams (CHTs) are clusters of public health staff assigned to neighborhoods to deliver community-based services. Teams are comprised of public health nurses, outreach workers and support staff that work with residents, families, and neighborhoods. CHTs are committed to being accountable and responsive to the community.

Teams work with residents to assess, develop and implement strategies that target health and safety concerns and the social needs of communities. This includes the identification and nurturing of community assets (schools, parks, churches, businesses, etc.) that enhance a community and can be tapped to address community concerns.

Currently, Community Health Teams partner with neighborhoods in:
    Livermore/Pleasanton
    Fremont
    Union City
    Hayward
    Ashland (unincorporated portion of San Leandro)
    East Oakland
    Fruitvale/San Antonio (Oakland)
    West Oakland

Friday Morning Meetings

CHTs and Health Department representatives host Friday Morning Meetings the 4th Friday of each month. Community residents, community organizations and representatives from other public/private agencies attend to discuss issues of concern.

  • Increase community awareness of community needs and strengths through a collaborative assessment process


  • Assist the community in prioritizing and promoting community capacity building


  • Enhance partnerships among the community, Public Health Department, and health service providers


  • Provide technical assistance for individuals and groups that want help developing projects


  • Train staff and community residents on community capacity building
