Nursing - Public Health
Alameda County Public Health Nurses provide leadership within systems and communities to achieve optimum physical, mental and social well-being for all residents. Public Health nursing encourages individuals and populations to take responsibility for their health.
Public Heath Nurses in Alameda County follow the American Nurses Association’s (ANA) Scope and Standards of Practice for Public Health Nursing (

). The emphasis is on the three core functions of assessment, policy development, and assurance. PHNs utilize the ten essential public health services

to guide their activities with individuals, families, and communities.
Public Health Nurses team with Community Health Outreach Workers to form 10 Community Health Teams (CHTs) in Alameda County that do the following:
Partner with the community to meet the community needs in the areas of health and safety
Provide nursing assessment, counseling, education and case management services to individuals and populations
Partner with public health staff and other agencies on issues related to maternal child health, tuberculosis, lead poisoning, Sudden Infant Death Syndrome, acute communicable disease, aging and adult health
Provide clinical services such as immunizations, flu shots, TB testing, and pregnancy testing to high-risk target populations
To improve the health of individuals and communities, Public Health Nursing has these goals:
Promote and maintain wellness among Alameda County residents through the development of community capacity, assessment, planning, intervention, and evaluation of factors affecting community health.
Provide case management and assessment services to high-risk women, infants and children, the elderly, and Medi-Cal recipients under 21.
Develop and utilize public health nursing resources such as assessment tools, acuity measurement instruments, nursing/community diagnosis, standardized interventions, and standards of care.
Develop an infrastructure for public health nursing practice including quality improvement, minimum competencies, staffing standards, and a minimum data set.
