HCPCFC addresses the health needs of foster children. Public Health Nurses work with Child Welfare Workers and Probation Officers to gather health information (medical, dental and behavioral) on each foster child to create a Health and Education Passport. This Passport travels with the foster child to the foster care provider so that health care is continuous for the child. |
| Program Eligibility: |
- All children in the Child Welfare system or Juvenile Probation system
- All children designated by the courts to be foster children

| Useful Information and Services: |
- Collection of relevant health data on each child in foster care
- Consultation with child welfare workers and probation officers on health plan for each foster child
- Education of foster parent and welfare and probation workers on health needs of child
