Homeless Families Program
The Homeless Families Program provides a non-duplicative and seamless system of care for homeless families in Alameda County. Homeless Families Program serves adults with minor children in their custody that are the hardest to reach and most challenging to serve.
Homeless Families Program provides comprehensive case management and support services that includes transitional and permanent housing assistance. Family Case Management is the core of Homeless Families Program and is a requirement in order to receive transitional or permanent housing assistance, as well as other support services.
The goal of this program is to link permanent, affordable housing with intensive case management and support services in order to break the cycle of chronic homelessness with multi-problem families.
Eligibility:
Families are eligible for the Homeless Families Program if they meet the following criteria:
• is currently homeless
• has legal and physical custody of at least one child under 18 years of age
• is willing to participate in case management services
Homeless families may contact the Office of the Day (OD) on scheduled shifts on
Monday, Wednesday and Friday from 9:30 a.m. – 12:30 p.m.
Health Care for the homeless case management staff maintains designated shifts via telephone where cursory assessments, support services and initial intakes are conducted for eligible homeless families. HFP staff will contact eligible homeless families to schedule an orientation and complete agency required forms.

Major Events and Activities:
Homeless Families Program provides six additional months of after-care for program participants after move-in to permanent housing.
