Public Health Nursing

phn1The Alameda County Public Health Nursing is staffed by public health nurses and community health outreach workers who work as a team to promote and maintain wellness among Alameda County residents. Individuals, families, and communities throughout Alameda County are provided a variety of services in their own home, clinic, community center, school, and other sites.

Services include:

  • general case management, health education and physical assessment
  • immunizations
  • information about health-related programs and preventive health care
  • assistance with scheduling medical appointments
  • information on transportation options and other support services
  • assistance accessing diagnostic and treatment services
  • referrals and support with Sudden Infant Death Syndrome (SIDS), communicable diseases, children with special needs, and children in foster care

The Alameda County Public Health Nursing Division provides public health nursing care through community outreach, home visiting, care coordination and advocacy to address individual and community health needs, promote healthy living, eliminate health disparities, improve health outcomes, and ensure optimal quality of life for all Alameda County residents.

School Initiatives: Health, Education and Attendance for Life (H.E.A.L.)
A health flyer for parents offers a checklist of symptoms/conditions and guidance on whether the student should go to school or stay home. The guidance is designed to promote health and reduce truancy.

Target audience

Low-income residents of Alameda County who are eligible for Medi-Cal; Child Health and Disability Prevention (CHDP)services; Maternal, Child and Adolescent Health Program (MCAH) services; and other programs and services for low-income Alameda County residents
