The Collaborative Community Planning Council (CCPC) of the Oakland Transitional Grant Area (TGA) has the mission to act as a catalyst to provide a continuum of high quality services for all people living with and affected by HIV disease and those at great risk for exposure to HIV/AIDS by empowering out communities, providing responsible stewardship and planning and advocating on our communities’ behalf.
Housing Opportunities for Persons with HIV/AIDS (HOPWA)

HOPWA s designed to increase affordable housing opportunities and stabilize the housing situations of people living with HIV/AIDS. Funds from the U.S. Department of Housing and Urban Development (HUD) are made available to local governments to provide grant and loan programs to assist low income residents with HIV/AIDS.
Routine Screening for HIV to Increase in Alameda County
Eight local community health organizations in Alameda and Contra Costa counties are working to increase the number of patients offered an HIV test as part of routine medical visits. These organizations have implemented universal, opt-out HIV testing into routine medical visits at their clinics and hospitals, aiming to test more than 50,000 people this year. Alameda County 2014 AIDS Housing Needs Assessment
The Alameda County 2014 AIDS Housing Needs Assessment (AHNA) Report, published February 2014, updates our community’s understanding of the housing and service needs of People Living with HIV/AIDS (PLWHA) in Alameda County. The AHNA Report presents its findings in the context of Alameda County’s AIDS housing policy and program history and builds on background information on housing and housing services for PLWHA.
The AHNA Report consists of two sections:

- AIDS Housing Needs Appendices (128 pages)
- 2014 AIDS Housing Needs Assessment Addendum
2013 HIV/AIDS Needs Assessment of People Living With HIV/AIDS in Alameda County
This Needs Assessment, completed by Facente Consulting, involved an extensive community effort over several months. Community input was obtain in a variety of ways all detailed in the report. The Assessment has been used in the Priority Setting and Allocations process that began May 2014.

Minority AIDS Initiative (MAI) 2013 Needs Assessment
The Alameda County Public Health Department’s Offices of HIV Care and Prevention 2013 Needs Assessment of People Living With HIV/AIDS in Alameda County was completed by Facente Consulting and involved an extensive community effort over several months. The Offices of HIV Care and Prevention's Positive Resources Directory is a 250-page comprehensive directory of local services, which address the needs of persons living with HIV/AIDS in Alameda County. In addition to providing information on HIV services, the directory also includes information about food programs, government benefits, shelters and other supportive services for that can benefit other low-income and/or disadvantaged populations.

Berkeley Free Clinic

The Berkeley Free Clinic is a clinic run almost exclusively by volunteers, with an emphasis on education, self-knowledge, and awareness of available public resources. The clinic's website has information about all of the services provided, appointment information, resources, as well as ways to volunteer and get involved.
