From the State Maternal, Child and Adolescent Health Program:

The statewide CPSP program provides a wide range of services to pregnant women from conception through postpartum. In addition to standard obstetric services, qualified Medi-Cal providers may receive additional reimbursement for providing enhanced services in the areas of nutrition, psychosocial and health education. Please visit the State Public Health CPSP website which includes the following documents:

  • Application for Certification as a CPSP Provider
    For Medi-Cal providers who want to offer the CPSP services to their patients. Look for form CDPH 4448
  • CPSP Provider brochure
    Program description, reimbursement, provider eligibility and application process.
  • Provider Handbook (updated 2016)
    Guidelines to setting up and maintaining a CPSP practice, delivering services to patients and complying with CPSP regulations.
  • Steps to Take Handbook (updated 2017)
    CPSP guidelines for providing health education, nutrition and psychosocial services. Contains dozens of reproducible, low literacy health education handouts in English and Spanish.

Prenatal Weight Gain Grids (Institute of Medicine)
