24/7 Communicable Disease Reporting For Clinicians

The Division of Communicable Disease Control & Prevention receives and responds to communicable disease reports. Physicians and health care providers, per Title 17 of the California Code of Regulations, are legally required to report suspected, lab-confirmed, and clinical diagnoses of specific diseases and conditions within specified time frames to the Alameda County Public Health Department.

Reporting Forms

• Communicable Disease Reporting Form
• CMR/A: Report all conditions except TB
• CMR/B: Report Tuberculosis
  Please use the Alameda County Public Health Department Confidential Tuberculosis form when reporting a tuberculosis case or suspected case (mandatory reporting within one working day).
• CMR/C: Report DMV conditions that impair consciousness or control
