Health Care for the Homeless Program (HCHP)

 
Alameda County Health Care for the Homeless (ACHCHP) is a mobile clinic-based medical and social services program. We provide health care to homeless persons throughout Alameda County. ACHCHP operates two mobile medical clinics and a stable-site clinic in downtown Oakland, offering medical and social services at 34 sites in Alameda County - in shelters, homeless centers, meal sites and other locations.

ACHCHP is a federally-funded 330(h) Health Care for the Homeless program that has been providing medical care to homeless persons since 1988. In 2009, ACHCHP provided services to 7,832 homeless persons.

ACHCHP staff includes three nurse practitioners, medical assistants, a Medical Director, six community health workers, a mental health specialist, and administrative and van support staff. Our sister organization is the Homeless Families Program, providing housing services to homeless families.

ACHCHP provides direct services and referrals to a wide range of medical providers, including the Alameda County Medical Center, the Alameda County Clinic Consortium, and other free, volunteer, community and mobile clinics. ACHCHP community health workers provide access to dental, optometry, specialized medical care, mental health, substance abuse treatment, food, shelters, legal and social services referrals, and case management services.

Our mission is to treat homeless persons with respect and kindness, providing them with the best quality health care possible, and provide access to comprehensive medical and social services, partnering with community resources in our common struggle to improve health and end inequality and homelessness.

 
Program Eligibility:

ACHCHP medical and social services are available to all persons who are experiencing homelessness or who are at-risk of homelessness. Shelter and program staff can find out what our monthly van schedule is by phoning 510-532-1930.

We answer telephone calls in our office at 510-532-1930, giving information and referrals by phone.

Frequently Asked Questions
Q. How can I get a motel voucher, food voucher or emergency assistance?
A. ACHCHP usually does not have funds for motel vouchers or emergency food assistance. You should phone 211 to get a list of agencies that may have emergency assistance available.

Q. Can I get help for a homeless person that I know or have seen on the street?
A. ACHCHP cannot come and ‘get people off the street.’ We can offer you information and resources, ideas of what you might do to engage and help someone you want to help. We’ll give you shelter contacts and resources and provide advice to help navigate the health and social services system.

Q. Where can I get a TB skin test if I’m not homeless?
A. We’re sorry - we can only offer free TB skin tests to persons who are homeless or in a recovery/transitional housing program. If you need a TB test for school, work, etc., and are not homeless, here is a link to a list of Alameda County TB skin test sites and a list of San Francisco TB skin test sites.

Q. Do you provide drop-in services in your office?
A. No, we do not. Only free TB skin tests every Tuesday 1-3pm for HOMELESS PERSONS ONLY. Please phone 510 532-1930 first
