California Department of Public Health has set up a website, www.calpanflu.org, for clinicians, health plans, hospitals, pharmacies and other entities interested in ordering and providing the H1N1 flu vaccine.Alerts and Forms
When testing, treating and reporting novel H1N1 (Pandemic H1N1 2009) influenza infections, use the Alameda County guidance and forms below. Case reports should be sent to our Acute Communicable Disease Unit by phone to 510-267-3250 or by fax to 510-268-2111, and not directly to the State.
